Return to contact sport is typically 9–12 months post-ACL reconstruction, when objective strength and movement tests are passed. Premature return is the most common cause of re-injury.

Complete ACL rehabilitation from injury to return-to-sport typically takes 9–12 months. The physiotherapy programme has distinct phases — each must be passed before progression.

Re-injury rate after premature return is very high. Patients returning at 6 months have a re-injury rate over 30%, compared to less than 5% for those returning after meeting objective criteria at 9+ months.

A complete ACL tear does not heal on its own, but with physiotherapy, many patients achieve stable, functional knees through strengthening compensatory muscles, especially the quadriceps and hamstrings.
