Morning heel pain is caused by the plantar fascia contracting overnight in a shortened position and then suddenly being stretched with the first steps. Exercises and night splinting prevent this effectively.

With physiotherapy, most patients see significant improvement in 6–8 weeks. Chronic plantar fasciitis (over 6 months) may take longer but responds well to shockwave therapy (ESWT).

Orthotics can provide pain relief by supporting the arch and reducing plantar fascia tension. However, exercises addressing the cause (calf tightness, intrinsic weakness) are the primary treatment.

Moderate walking is fine and often recommended. Prolonged standing and barefoot walking on hard floors should be avoided. Supportive footwear at all times — including in the house — is essential.

Shoes with good arch support, cushioned heel, and firm midsole are ideal. Avoid flat shoes, flip-flops, and bare feet. We provide specific footwear recommendations.

Running is often too aggravating in the acute phase. We modify your training load and introduce a graded return-to-running programme once the fascia can tolerate it.

Rarely. Over 95% of plantar fasciitis cases resolve with conservative treatment including physiotherapy, stretching, and footwear modification. Surgery is a last resort.

Bilateral plantar fasciitis is less common and may suggest a systemic cause (rheumatoid arthritis, spondyloarthropathy). We screen for these and refer appropriately.
